Gather evidence of how each support helped you reach your goals List any changes in your circumstances (health, living situation, work, study) Note supports that worked well and any that didn't Check your remaining budget by category Write down new or updated goals for the next plan period Get input from your support coordinator, family or support workers Collect any new reports (therapy, medical, allied health) if relevant Book your plan review meeting or check for an outreach date
