Provide day-to-day personal care, community access, and life skills support to NDIS participants across Perth.
Support participants to engage with their community, attend appointments, social activities, and build independence.
Deliver high intensity nursing supports including wound care, medication management, tracheostomy, PEG feeds, and complex care plans.
Work alongside RNs and support workers to deliver delegated nursing tasks and participant-centred clinical care under the NDIS.
Help participants implement and maximise their NDIS plans by connecting them to services, resolving barriers, and building capacity.
Lead a team of support workers, manage participant service delivery, handle scheduling, and ensure quality outcomes on the ground.
Support the operations team with participant onboarding, compliance documentation, scheduling coordination, and office administration.
Manage staff rosters, match workers to participants, handle last-minute changes, and ensure continuity of care across all services.
Flexible casual and on-call roles for experienced support workers available to cover shifts across Perth. Great for those balancing other commitments.
